How To Use Ultrasnow 1.2.8 To Unlock iPhone
In order to unlock iPhone 4 or 3GS you must first confirm the baseband and Ultrasnow unlock status of your phone.
Only proceed when you know and are very sure that your baseband can be unlocked via Ultrasn0w.
If you are good to go, follow the instructions here to jailbreak your iPhone and install Cydia.
After you have installed Cydia, connect your jailbroken iPhone to your wireless internet hotspot and let us rock and roll.
Place your new sim card in the iPhone tray and Tap the Cydia app in your jailbroken iPhone.
If you have successfully connected to the internet, Cydia should open without a fuss. Select the Settings and then Manage tab. You should see a page as shown on the right.

How To Use Ultrasnow Fixer
I am assuming that you are using IOS 6.0 or 6.1 and getting locked out of your iPhone.
Go ahead and install Ultrasn0w Fixer in order to enable the unlock.
In order to install Ultrasn0w Fixer, head over to Cydia and start by installing a repository called iParelhos. Follow the instructions below in order to do this:
Tap Manage and then Sources.
Type http://repo.iparelhos.com
Search for Ultrasn0w Fixer 6.0.1 or 6.1 and install it.
Search and install Ultrasn0w 1.2.8 from Cydia.
Restart Cydia and iPhone, and you should now see the network signals.
Your iPhone is now jailbroken and unlocked and it should support virtually any sim card thrown at it.

2 How To Unlock An iPhone Using Gevey Sim Interposer
Gevey sim unlock has been around for quite some time. For long, it was the best alternative to Ultrasnow unlock, and the only solution where Ultrasnow failed.
Another advantage, or disadvantage, depending how you look at it, gevey sim unlock saves you from having to jailbreak your iPhone. An iPhone unlocked via gevey interposer works out of the box with any carrier sim.

Unfortunately, Gevey sim unlock is only possible up to iPhone 4 and iPhone 4S. iPhone 5x cannot be unlocked via Gevey sim tray.
What Is Gevey Sim Card Unlock?
Gevey sim unlock is a hardware based interposer to unlock iPhone 4 or any other iPhone.
A gevey sim tray sits between the hardware baseband of the iPhone and the carrier sim and performs a Man In The Middle Attack (MITM).
It is a sim tray which takes advantage of a few exploits and makes your new sim relevant to a carrier locked iPhone.

3 How To Unlock An iPhone Using R-Sim Card Interposer
R-sim interposer is a popular to unlock iPhone 4S through to iPhone 5x. It seems to have taken over where Gevey sim tray stopped.
R-sim 7, 8 and 9x versions will also unlock IOS 7x.
Other than plain unlocking, this sim interposer can also be used to activate stubborn iPhones, especially from Sprint.
Unlocking the iPhone with newer r sim interposer is as simple as placing your new macro sim (iPhone 4 or 4S) or nano sim (for iPhone 5) onto the interposer, which comes with its own tray.
Slide the tray into the iPhone and follow the simple instructions that follow. You will be prompted to restart the iPhone to complete the unlock process.
You may may however encounter issues when activating and unlocking Sprint based iPhones.
Whereas the voice calls will function normally, SMS and data may not respond in your iPhone.
